Q. Do I need a referral from my physician to schedule an appointment with a therapist?
A. You need an order or prescription from your physician to start therapy. If your physician has recommended therapy, but you don’t have a written physician order, call our scheduling line at 612-273-2896 for assistance in obtaining written orders.
Q. Our physician recommended my child for pediatric rehabilitation. How do I schedule an appointment?
A. Call central scheduling at 612-273-2897. A parent or legal guardian must accompany the child to the initial appointment.
Q. How do I know if my insurance covers therapy?
A. You are responsible for calling your insurance company to find out what coverage you have. You are also responsible for notifying Fairview of any changes in insurance.

Q. What can I expect at the first appointment/ evaluation?
A. The therapist will discuss your reasons for being referred to rehabilitation. The therapist may observe your child in play or perform specific tests, or both. The results of this evaluation will be reviewed with you. Goals and a treatment plan to help your child reach those goals will be developed based on your concerns as well as the therapist’s evaluation. Parent participation is critical at the evaluation. Because you may be actively involved in the evaluation, we recommend you and your child attend the session alone. Another person should care for siblings. The evaluation should take about 60-90 minutes.
Q. What should I bring to an evaluation?
A. You should bring the following to the evaluation, if you have them: · A completed Child Developmental History Questionnaire. If your child receives services with your local school district, bring any pertinent school reports such as an Individual Education Plan (IEP) · Other recent evaluation reports from physicians, neuropsychology, etc · If your child is being seen for an orthopedic injury or assessment such as a knee or shoulder injury, please bring shorts or a tank top so the therapist can easily observe joint movement
