Rehabilitation diagnoses
Some of the medical conditions that may be appropriate for referral to inpatient acute rehabilitation include: 

  • Stroke
  • Hip fracture
  • Spinal cord injury
  • Brain injury –traumatic and atraumatic
  • Polyarthritis
  • Congenital deformity
  • Amputation
  • Major multiple trauma
  • Other neurological disorders (i.e. MS, movement disorders, Parkinson’s)
  • Complex medical
    • Transplant
    • Cancer
    • Other
  • Bilateral joint replacements
  • Single joint plus
    • age 85+
    • BMI 50+
    • Complex medical

Level of service required
M
ust have needs in the following areas during stay

  • Comprehensive rehabilitation
  • Rehabilitation nursing
  • Two or more therapies (PT, OT, ST)
  • Two or more of the following areas:
    • ADL, IADL, home/lifestyle modification
    • Bed mobility/transfers
    • Speech & language retraining, swallowing
    • Cognitive retraining
    • ROM, strengthening
    • Positioning, splinting
    • W/C mobility, gait, balance

Medical management
One or more of the following:

  • Skin care
  • Medications
  • Bowel and bladder
  • Respiratory issues
  • Pain or spasm
  • Nutritional issues
  • Coping skills
  • Adjustment to loss
