Getting Started
You will need a release of information authorization form so you can participate in MyChart. If you have questions about filling out the form, contact your clinic’s MyChart representative. When the clinic receives your release form, you will be mailed your startup information. It will provide you with:

• Your access code
• Answers to frequently asked questions
• A guide to compatible computer and Internet operating systems
• A reference guide for software operation
