Falling is the leading cause of death from injury among older adults, and falls are among the most common causes of hospitalization. Falls can be prevented. That is why each staff member is specially trained to help keep you from falling.

You may call us at 612-728-2468. One of our staff will then contact your physician to obtain a referral.
We will work with you and your physician to determine how home care staff may assist you. Our staff will also help you with insurance questions and verify coverage. Fairview Home Care and Hospice is Medicare-certified and accepts all major insurance plans and private payment.
The assessment and survey
The assessment and survey identifies ways to help keep you safe at home. Here are just some of the things we look at with an eye for your safety:

- Your strength
- Your balance
- Lifestyle choices that affect the risk of falling
- Medication management issues
After the assessment
We write a plan specifically for you with recommendations on what and who can help you to avoid falling. The plan also includes actions for those involved in your health care, including your physician, home care staff and caregivers.
