Online: Use the online transfer form and choose "Mail Service" from the top of the locations menu. NOTE: Prescription transfers cannot be submitted at the myfairviewrx website.
How do I get additional refills through the Fairview Mail Service Pharmacy?
By Mail: Mail back the pre-printed order form that comes with each shipment.
By phone: Call 1-866-377-6245.
Online: Go to myfairviewrx, click "Quick refill" and choose "Get my prescription by Mail Prescription Order" (not "Store Pickup). Or, register for an account for additional features such as automated refill reminders. NOTE: Each family member requires a separate email address to log in.
How is confidentiality ensured?
Your medication will be sent to you in a plain package with a return address only.
The pharmacy name will not be listed on the package.
How do I pay for the service?
Copayments may be made by credit card. Credit card information must accompany
your prescription request.
How long will it take to receive medications through the mail service?
You can expect to receive your medications 3 to 5 days from the time we receive your valid prescription order.
Is there a charge for postage?
For regular mail delivery, there is no charge for postage.
