You are the most important member of your health care team, and you deserve to participate fully in the care you receive at Fairview. We want you to feel confident in making important decisions about your health care.
Fairview created this Web site to help you understand how our care providers are working to fulfill our promise that your care at Fairview will be safe, effective, patient-centered, timely, efficient and equitable. Each section will show how we measure care for the six "aims" established by the Institute of Medicine. Our measures are based on scientific evidence, often in collaboration with state and national quality and safety initiatives.
Meeting treatment goals
Many of Fairview’s quality care measures include tracking how many patients achieve all treatment goals for their disease or condition when they visit a Fairview clinic or hospital. Found under the effective care section of this site, these measures look at such diseases and conditions as asthma, diabetes, heart attack and pneumonia, among others.
We follow specific steps for each patient who comes to Fairview with one of these diseases or conditions. For example, providers ask adults with diabetes if they take an aspirin every day and whether or not they have had a hemoglobin A1c test within the past six months, among other measures.
Get involved in your care
Use this Web site to continue learning about safety and quality efforts at Fairview and at other health systems in Minnesota. Find out how we compare to other hospitals. Fairview also works with local and national organizations to enhance patient safety. See the full list of organizations. One such organization, Joint Commission, conducts surveys at hospitals nationwide to evaluate whether or not they are meeting specific performance standards. Joint Commission accreditation is recognized as a symbol of quality; all Fairview hospitals are accredited.
