Fairview Hospice participates in Quality Partners, a national initiative to improve hospice and palliative care.

When life expectancy is limited, Fairview Hospice staff and volunteers provide comfort care and focus on improving the quality of remaining days. With our help, you and your family can focus on improving the quality of your days, rather than on the symptoms and stresses of your advanced illness. Hospice provides medical, emotional, social and spiritual care, all in the familiar setting of your own home whenever possible. The same kind of care may also be provided in assisted living, group home, nursing home or other inpatient setting.

When is Hospice Appropriate?
Hospice services are usually provided when life expectancy is less than six months.  Hospice is appropriate for individuals with advanced cancer,
cardiac disease, AIDS, pulmonary disease, neurological conditions and other chronic diseases.

What does Hospice provide?
The goal of hospice is to minimize pain and discomfort while offering guidance and support to you and your family.  We supplement the loving care your family provides in a variety of ways:

  • Regular visits from registered nurses with advanced training in pain
    control and symptom management
  • On-call nurses available 24 hours a day, 7 days a week
  • Hospice aides to assist with bathing and other personal care needs
  • Medications, medical supplies and equipment needed at home
  • Social workers to provide supportive counseling and other assistance
  • Visits from spiritual care providers respectful of your family’s faith and belief system
  • Trained hospice volunteers who provide a comforting presence for you or respite for your family
  • In a medical crisis, around-the-clock nursing care at home or short-term hospitalization
  • Thirteen-month bereavement support for your family members and loved ones
  • Support for the entire family

Working together with you, we provide care that's specific to your needs and the needs of your family or caregiver.  Our hospice team includes:

  • You and your family
  • Hospice aides
  • Your physician
  • Spiritual care providers
  • Medical directors
  • Volunteers
  • Nurses
  • Massage therapists
  • Pharmacists
  • Music therapists
  • Social workers
  • Bereavement counselors
  • Dietitians

Costs are covered
Hospice costs are covered by the Medicare Hospice Benefit, the Medical Assistance Hospice Benefit and most private insurance plans. Our social worker can help you obtain coverage from your particular insurance plan.
