Twelve Fairview primary care clinics were recently recognized by the Buyers Health Care Action Group (BHCAG), for improving the care of patients living with diabetes, artery and vein disease and depression.
A coalition of more than 30 private and public employers, BHCAG’s annual Bridges to Excellence program evaluates primary care clinics in the metro area for their ability to achieve high patient outcomes scores in key clinical areas. Bridges to Excellence is in its third year and has recognized Fairview Clinics in the past for initiatives in the areas of diabetes care, coronary disease management and coronary artery disease.
Fairview clinics recognized by BHCAG in 2009 include:
• Diabetes care: Fairview Maple Grove Medical Center, Fairview Oxboro Clinic, Fairview Rogers Clinic;
• Vascular (artery/vein) care: Fairview Eagan Clinic, Fairview Hugo Clinic, Fairview Jonathan Clinic (Chaska), Fairview Oxboro Clinic, Fairview North Branch Clinic, Fairview Ridges Clinic (Burnsville) and Fairview Ridge Valley Clinic (Apple Valley); and
• Depression: Fairview Chisago Lakes Clinic, Fairview Highland Park Clinic (St. Paul), Fairview Lakeville Clinic and Fairview Rogers Clinic.
