Fairview Home Care and Hospice Community Care Program

We want to help you if you are unable to pay all or part of your medical bill. Fairview Home Care and Hospice’s community care program is part of our mission to provide quality medical services to everyone in our community. We treat all of our patients with dignity and respect, regardless of their ability to pay for our services. If you have any concerns about your bill or cannot afford to pay, please call us.

If you do not have insurance or if your insurance does not cover all your costs, you can apply for help through our community care program. Whether you are eligible for the community care program will depend on several things, including how much money you make.

We understand that there may be many reasons why you are unable to pay for medical care. Whatever the reason you still need medical care, and the Fairview’s community care program can provide financial help for the treatment you receive.

Fairview Home Care and Hospice’s community care covers charges for most of our services. However, it does not cover charges for:

Products or services that are not medically necessary.
Services your insurance will cover from another home care agency, but not from Fairview Home Care and Hospice.
Services provided in a setting other than the patient’s residence.
Services provided to people who are not a Minnesota resident.

If you do not know whether the care you are seeking is covered by community care, please ask us.

The income limits that would qualify and the level of coverage are as follows:
